Get your Chicago trip information to your instructors by Friday, April 9

1. Ad majors and first-year class one

Please provide the following information to Kenton by Friday, April 9 at noon:

1. Name (first, last)
2. Birthdate (month, day, year)
3. Country of citizenship
4. Photocopy of border ID – either a valid (not expired!) passport or enhanced license from MPI. Photocopy the part with your photo/number.

2. PR majors and first-year class two and three

Please provide the following information to Melanie by Friday, April 9 at noon:

• Name (first, last)
• Birthdate (month, day, year)
• Country of citizenship
• Photocopy of border ID – either a valid (not expired!) passport or enhanced license from MPI. Photocopy the part with your photo/number

You can hand this in to our shelves on the third floor or by email (but you’ll need to scan in or take a clear digital photo of your documentation to do it).

No border ID means that you won’t be able to go along on the trip. Get your ID today!
